Client Agreement – Code of Care & GDPR Policy

1. Introduction

Welcome to Down Days Hypnotherapy. This agreement outlines our commitment to you and your responsibilities as a client. It also explains how we handle your personal data in accordance with the General Data Protection Regulation (GDPR).

By engaging in hypnotherapy services with Down Days Hypnotherapy, you agree to the terms outlined below.

2. Code of Care

As your hypnotherapist, I commit to:

  • - Providing a professional, confidential, and safe therapeutic environment.
    - Treating you with respect, empathy, and without judgment.
    - Practicing within the scope of my qualifications and referring you to other professionals when appropriate.
    - Staying current with ongoing professional development and ethical practices.

A full copy of the Association for Solution Focused Hypnotherapy’s (AfSFH) and/or the National

Council of Hypnotherapy’s (NCH) Code of Practice is readily available.


 As a client, you agree to:

  • - Attend sessions punctually and provide at least 24 hours’ notice for cancellations.
    - Participate actively and honestly in the therapeutic process.
    - Disclose any medical or mental health conditions that may affect your sessions.
    - Understand that hypnotherapy is a collaborative process and not a guarantee of results.
  • It is advisable to discuss any health issues with your GP 

3. Confidentiality

  • - All sessions are strictly confidential. No information will be shared without your written consent, except where required by law (e.g., risk of harm to self or others, or legal obligation).
    - Session notes are kept securely and accessed only by your therapist.

4. GDPR – Privacy and Data Protection

Personal Data Collected:

  • - Full name, contact details, date of birth.
    - Medical history and presenting concerns (where relevant).
    - Session notes and correspondence.

Why We Collect It:

  • - To provide safe and effective hypnotherapy services.
    - For scheduling, invoicing, and administrative purposes.
    - To comply with legal and regulatory obligations.

How We Store It:

  • - Paper records are securely locked.
    - Digital records are stored in encrypted files and/or password-protected systems.

How Long We Keep It:

  • - Records are kept for 7 years after the last session (or until the client reaches 25 years old, if under 18), after which they are securely destroyed.

Your Rights:

  • - Access your personal data.
    - Request correction or deletion of your data.
    - Withdraw consent to processing.
    - Lodge a complaint with the Information Commissioner’s Office (ICO).

5. Fees, Cancellations & Refunds
- Cancellations with less than 24 hours' notice may be charged in full.
- In the event of a refund, it will be considered on a case-by-case basis.
